Azabu Tailor (Shinjuku and Ikebukuro branches, at least) does a pretty good job for a decent price.
I've had two suits from them and their service was excellent.
But I'm not that particular about my suits, so your mileage may vary.

Azabu Tailor - Whilst the price and olde-world-gentleman’s-club style of their stores seems very attractive, I have found the quality (and more so longevity) is not up to snuff if you’re used to MTM tailoring elsewhere in Asia. Despite trying for years I still haven’t found a real quality suits tailor in Tokyo (without dropping ¥200-300k at Mitsukoshi) and continue to get all my suits made bespoke in Bangkok.
For shirts however can highly recommend Makers Shirts / Kamakura Shirts - bit more expensive than Azabu Tailor but construction and materials far higher quality, especially if you go for the imported fabrics.

I can only give “recommendations” for tailors I’ve tried and don’t like.
I was back in Hong Kong last month and got really high quality shirts made up in four days at one third of the cost of the poorer quality ones I’ve had made in Tokyo.
Thanks for the recommendation for Kamakura Shirts, I’m set for a few years now but will try them next time.

Good value to be had there.

Is that fully bespoke or sewing together panels? What is the price range?
I have to say I was very disappointed with Isetan when I had a suit made up for my wedding many many moons ago. It was a pretty poor fit, even after a round of adjustments. I really thought they didn’t really have much experience with Western body sizes.

I know a taylor who comes to Tokyo and Osaka from Hong Kong about every 2-3 months, and then takes the work back to Hong Kong. I think his suits are good.
I'll let you know when I know he's coming to Japan next.
Price depends on material chosen, optional extras, etc..

I recommend Barons at Kamata (KeihinTohoku line, also Tokyu), have been going there for 30 years.. The shop has not changed much over the years but very nice quality for the price, and service is impeccable. Even when one jacket needed adjustment many years later, they gladly did it

Not fully bespoke. That costs much more. 30,000ish for cloth on sale. Often 50,000 for 2 suits. This is for the sale cloths. Brand-names are more expensive, of course.
I had some made at Matsuya and Mitsukoshi. Happy enough with them and still wearing them 15 years later.
