Schwab card--use it for car rental and you can decline the extra insurance at the rental desk. Buy your air ticket with it and some travel insurance comes with that. (No specifics on amounts, but stuff like that is in the card agreement pamphlet.) No 'points', tho, if it's that kind of thing attracts you.

I think there are a reasonable number of US cards that offer car rental coverage.
