This is sort of what I was getting at when I asked whether the employer wanted OP to contribute more than usual.

Good to hear that the employer is conscientious and that the misunderstanding has been resolved -- and that you can even wait until after your leave to pay.

I just read this for the first time from the Jetro link posted above, anyone knows how it works/ever used it? Do we only need to send foreign medical receipts to our work insurance company or the city hall NHI section?
I always thought medical expenses incurred abroad would never been taken into account in Japan… My fault for never looking into it ><

“Benefit: Overseas expenses
If an insured party incurs medical treatment expenses at a medical institution while staying or traveling overseas, he/she can apply to be reimbursed after returning to Japan. The amount of medical expenses incurred overseas is converted into a comparable amount of Japanese medical expenses, and 70% of that amount is reimbursed. It should be noted that this also applies to foreign nationals insured under this system who receive medical treatment in their own countries or other countries outside of Japan.”
