I've run into this a few times.
A lot of forms seem to have two fields, "application date" and "start date" or similar.
This time, a company sent me a form with the start date pre-filled to 1 December, but with the application date empty and a post-it saying "please fill out 1 December or earlier". Well... it's way past the 1st December, and I have to inkan next to it, so that seems like a false declaration. I feel like they are asking for this because it's much simpler administratively if we'd all agreed this before the service started, but that's not the case and they're asking me to lie to cover themselves.
Anyway in the past I've usually just complied, but this time I ignored it and put today's date. Lets see what happens.
Does anybody else have experience with this?

I've run into this too. To protect your interests, it would be better to ask the issuer to reissue the document. Unless, of course, it means you missing out on a job...
